چکیده

Background Stunned myocardium reflects postreperfusion dysfunction in myocardium that is destined to ultimately fully recover. Most investigators attribute postreperfusion stunning to a primary defect in excitation-contraction coupling or to an altered sensitivity of the myofilaments to calcium. The aim of the present study was to evaluate the interrelation between myocardial perfusion, oxidative metabolism, and function in an effort to better characterize the phenomenon of myocardial stunning, to define the regional efficiency of stunned myocardium, and to characterize its reserve capacity. Methods and Resuks Regional myocardial perfusion (measured with radiolabeled microspheres), myocardial oxygen consumption (MVo2) (quantified with positron emission tomography using 1-"C-acetate), and myocardial function (assessed with two-dimensional echocardiography) were evaluated in 12 anesthetized, closed-chest dogs subjected to 15 minutes of left anterior descending coronary artery occlusion followed by reperfusion. To evaluate flow, oxidative, and functional reserve after measurements were obtained 1 hour after reperfusion, dogs were subjected to paired pacing (an inotropic stimulus that does not alter systemic hemodynamics), and measurements were repeated. One hour after reperfusion, stunned myocardium was characterized by near-normal levels of myocardial perfusion (0.57±0.13 mL/g per minute, 81±13%

Diffuse bilateral breast uptake on the myocardial perfusion SPECT of a nursing female

Bilateral diffuse intense breast uptake was noted in a 40 year old female who was evaluated with Tc-99m sestamibi (MIBI) myocardial perfusion scan for possible ischemia. She was breast feeding her 1.5 year old child. The intense uptake in the breasts was superimposed on the apical and anteroapical regions of the myocardium and caused interpretation problem. We recom...

Reverse redistribution in the myocardial perfusion scan with ²º¹Tl [Persian]

Reverse redistribution pattern is defined as decreased activity in the myocardium in the rest phase of the myocardial perfusion scan in comparison with the stress images. There are many studies concerning the etiology and clinical significant of this phenomenon in nuclear medicine literature. The dominant idea about etiology of reverse redistribution is early wash out of the radiotracer f...

Pulsed wave tissue Doppler imaging for the quantification of contractile reserve in stunned, hibernating, and scarred myocardium.

OBJECTIVES To assess whether quantification of myocardial systolic velocities by pulsed wave tissue Doppler imaging can differentiate between stunned, hibernating, and scarred myocardium. DESIGN Observational study. SETTING Tertiary referral centre. PATIENTS 70 patients with reduced left ventricular function caused by chronic coronary artery disease. METHODS Pulsed wave tissue Doppler i...

Contractile reserve and coronary blood flow reserve in collateral-dependent myocardium.

Numerous studies in recent years have clearly demonstrated that viable myocardium with contractile dysfunction maintains a contractile reserve that can be unmasked by inotropic stimulation, such as by the infusion of low doses of dobutamine. Hence, low-dose dobutamine echocardiography (LDDE) has become firmly established as a diagnostic tool in patients with coronary artery disease (CAD) and le...
